About Safety Net Connect
With a mission of improving quality, efficiency, and communication through the use of innovative technology, Safety Net Connect has successfully reached over 10 million low-income, underserved patients in partnership with public and private healthcare systems.
Our patient-centered solutions span the care continuum, including: health plans, hospitals, community health centers, public health departments, correctional health systems, as well as skilled nursing and long-term care facilities. This diverse experience has helped shape our vision and the development of affordable, effective technology solutions based on four fundamental principles:
- Facilitate access for underserved and underinsured populations.
- Track activity to promote transparency, enhanced oversight, and measurable outcomes.
- Share information to improve communication and care coordination.
- Streamline the referral process using real-time, evidence-based data.
The Team
Safety Net Connect (SNC) was founded by leaders in health information technology who believed that an innovative systems-based approach could solve many of the nation’s fundamental healthcare problems.
In 2009, two pioneering health tech companies, NetChemistry and ELM Technologies, joined together to form SNC to utilize the potential of healthcare information technology for underinsured and underserved “safety net” populations.
The SNC team includes a diverse staff of thought-leaders and skilled professionals with a breadth of experience in healthcare, technology, consulting, customer service, and training. We believe that implementation planning, execution, and user adoption are critical elements for long-term success.
A dedicated professional services team collaborates with each client — providing hands-on guidance to ensure each implementation is tailored to an organization’s unique needs and culture, and that each organization achieves its goals for successful use of our solutions.
